Forms Processing Changes

As a result of limitations/restrictions on person-to-person contact imposed on campus to combat the Coronavirus outbreak, OGAPS has implemented some changes in the way we process documents. Our goal is to minimize person-to-person contact while continuing to process documents in a timely manner, avoiding disruptions and delays. We are now accepting electronic submissions of all forms. Please be sure to follow the instructions, as failure to do so could result in processing delays. If you encounter any situation that prohibits you from utilizing the revised processes for securing approvals or submitting documents, please contact OGAPS at ogaps-info@tamu.edu.

Research Proposal Approval Form
Instructions. (1) The student should complete the first page of the form and then send to the committee chair. (2) The committee chair should complete the second page, listing the names of the committee members and the Department Head or Intercollegiate Faculty Chair, signifying that all listed approve the Research Proposal. (3) The committee chair then sends to delegated departmental staff or authorized signer, who in turn should submit the completed form to ogaps@tamu.edu from their official TAMU email address.
 
Preliminary Examination Checklist and Report
Instructions. (1) The student should complete the first page of the form and then send to the committee chair. (2) The committee chair should complete the second page, listing the names of the committee members and the total tally of their votes. (3) The committee chair then sends to delegated departmental staff or authorized signer, who in turn should submit the completed form to ogaps@tamu.edu from their official TAMU email address.
 
Request for Final Examination
Instructions. (1) Departmental members (e.g., graduate advisors, committee chairs) should complete the form. (2) From their official TAMU email address, the delegated departmental staff or authorized signer should submit the completed form to ogaps@tamu.edu at least 10 working days prior to the defense date.

Final Exam Results 
After ensuring the student has met the requirements to hold the Final Exam, the Office of Graduate and Professional Studies will email the committee chair and Department Graduate Advisor the Final Exam Results Form. The committee chair (1) should complete the form, including the votes from each committee member or substitute. (2) The committee chair then sends to delegated departmental staff or authorized signer, who in turn should submit the completed form to ogaps@tamu.edu from their official TAMU email address.
 
Request for Exemption from the Final Examination
Instructions. (1) The committee chair should complete the form, including the names of each committee member. (2) The committee chair then sends to delegated departmental staff or authorized signer, who in turn should submit the completed form to ogaps@tamu.edu from their official TAMU email address.

Thesis, Dissertation, and Record of Study Copyright and Availability Form
Instructions. Students should complete the form and email to ogaps@tamu.edu. The committee chair will be asked to approve the Copyright and Availability selections of the student when acknowledging the document in Vireo.

Letter of Intent
Instructions. (1) The student should complete their information on the form and then send to the department head or intercollegiate faculty chair of the admitting department/program. (2) The department chair or intercollegiate faculty chair of the admitting department/program should complete their information to indicate approval. (3) Either the department head/intercollegiate faculty chair or a delegated departmental staff or authorized signer of the admitting department/program submits the completed form to ogaps@tamu.edu from their official TAMU email address.
