Graduate and Professional Studies Forms

Graduate and Professional Studies uses DocuSign for a number of forms. On this page, you can find helpful tips on how to use DocuSign, links to the forms, and instructions for each form.

DocuSign Tips

  • See the step-by-step video to show how DocuSign works.
  • Log into DocuSign to link your TAMU account before submitting any forms. Logging in will automatically link the account with your NetID and will allow you to later check the status of documents which you have submitted, signed, or are awaiting your action.
  • With feedback from the Texas A&M community, OGAPS has developed workflow processes for each form.
    • The Initiator (either student or Graduate Program Assistant, depending on the form) starts the form and completes the requested information.
    • If you are in the Workflow, you will receive an email to your TAMU email account. Once you receive an email, respond to the prompts, provide your initials or signatures (whichever is requested), and then continue.
  • You can check the status of any forms submitted after you have linked your Docusign account with your NetID by signing into DocuSign using your TAMU email. See the overview video and steps for more information. 
  • Follow the "Using DocuSign" guide for your role:
  • DocuSign has a robust Support Center. You can watch a video to learn more about signing documents. There are also step-by-step instructions related to printing documents, accessing documents you previously signed, and other basic information

Graduate and Professional Studies Forms

IMPORTANT: Log into DocuSign before initiating any forms. Logging in will automatically link the account with your NetID and will link your credentials with the forms you submit. After logging in, log out and complete the required forms below. Additional instructions are available here.

Research Proposal Approval Form

  • Initiator: Student
  • Directions: Students should initiate this form. Before proceeding, you must identify the name and TAMU email for the (1) Graduate Program Assistant in your department, (2) your committee chair, (3) your committee members, and (4) the Department Head or Intercollegiate Faculty Chair (or their designee). Complete all information applicable information, click “Begin Signing,” and complete all requested fields on the form.  
  • If you require assistance entering the requested information, please contact your Graduate Program Assistant.

Preliminary Examination Checklist and Report

  • Initiator: Student
  • Directions
    • Students should initiate this form. Before proceeding, you must identify the name and TAMU email for the (1) Graduate Program Assistant in your department, (2) your committee chair, (3) your committee members, and (4) the Department Head or Intercollegiate Faculty Chair (or their designee). Complete all information applicable information, click “Begin Signing,” and complete all requested fields on the form. 
    • The Graduate Program Assistant will then review the requirements checklist and either (1) confirm all requirements are met and initial the form, or (2) void the form if requirements have not been satisfied. If the latter occurs, the Graduate Program Assistant should alert the student of the changes needed.
    • If the requirements on the checklist are met, the Department Head or Intercollegiate Faculty Chair (or designee) will sign.
      The form will then route to the committee members, who wait until the exam is held to report the results.

Request and Announcement of the Final Examination

  • Initiator: Student
  • Directions: Students should initiate this form. Before proceeding, you must identify the name and TAMU email for the (1) Graduate Program Assistant in your department, (2) your committee chair, and (3) the Department Head or Intercollegiate Faculty Chair (or their designee). Complete all information applicable information, click “Begin Signing,” and complete all requested fields on the form.

Request for Exemption from the Final Examination

  • Initiator: Graduate Program Assistant
  • Directions: Graduate Program Assistants should initiate this form. Complete all information applicable information, click “Begin Signing,” and complete all requested fields on the form.  

Written Dissertation Approval Form

  • Initiator: Student
  • Directions: Students should initiate this form. Before proceeding, you must identify the name and TAMU email for the (1) Graduate Program Assistant in your department, (2) your committee chair, (3) your committee members, and (4) the Department Head or Intercollegiate Faculty Chair (or their designee). Complete all information applicable information, click “Begin Signing,” and complete all requested fields on the form.

Written Thesis Approval Form

  • Initiator: Student
  • Directions: Students should initiate this form. Thesis, Dissertation, and Record of Study – Copyright Availability

  • Initiator: Student

Letter of Intent

  • Initiator: Student
  • Directions: Students should initiate this form. Complete all information applicable information, click “Begin Signing,” and complete all requested fields on the form.
